Well the Patrol does have to prioritize the life of the reader first, since they are considered the most value member of a patrol. No reader then demons can sneak up on a group, and readers aren’t common to come by.
Though yes Sil is partial to blame for Gale’s death, but it is very minor, and possibly inconsequential. At worse it was the early camping, but Sil shouldn’t be blaming herself because she pointed out the demon. It is her job to do exactly that anyways. The patrol needs to know of demons close by so they aren’t caught off guard. It is largely Bernin’s fault for charging in head first and ignoring Cormac. Also no one in the patrol knew Bernin wasn’t going to be in a life or death situation. They just had to take it at face value.
Though Sil is feeling overly guilty in Gale’s death is understandable all this may be a problem in the future. If Sil starts to second guess about calling out demons in patrols in the future it may put the patrol in danger from ambushes, which can cost more lives. If she hesitates in supporting her friends/allies that may be bad too.
In the end Bernin probably learned that he shouldn’t be so bull headed. Sil on the other hand if she is wrapped up in guilt and doubt it may be a detriment to her and her team.
In the end, I’m afraid that the higher ups will consider this loss as Cormac’s fault. As he was in command at the time and responsible for making sure that everyone followed protocol, and more importantly, FOLLOWED HIS ORDERS, he failed as a leader because he couldn’t keep his patrol in line. Gail didn’t follow his order in the fight, Sil disobeyed him, Bernin well, we know what Bernin did. Even Callum made a mistake, but it’s also Cormac’s job to keep them from MAKING mistakes in the first place. The High Command will put the responsibility for this mess, the BLAME, squarely on his shoulders.

Even if this is mostly Bernin’s fault the full blame of the loss of Gale will fall on Cormac in the eyes of his superiors.
With Sil already feeling guilty about this – this will only make it worse as it will be Cormac answering, and taking the blame for what happen. Heck depending on what Hillel puts in his report to the council Cormac might be demoted or even transferred. I’m sure casualties from a patrol isn’t unheard of, but what matters is the events that transpired, and based on Hillel’s current attitude it may not look good for Cormac.
